All office visits are out-of-network with all insurance companies and payment is due at the time of service. I can provide you with a super bill, containing all the codes needed to bill your insurance company for reimbursement. All lab work and diagnostic imaging are billed directly to your insurance company.
Billing
Intrauterine Insemination (IUI)
Fees: for established patients only
IUI with frozen sperm: $450
IUI with fresh sperm (including a sperm wash): $550
Second IUI in the same cycle: $300
IUI with Zymot Device: $750
Weekend: + $50
Holidays: + $100
Late cancellation (less than 4 hours before), and no-show fee: $300
